It doesn't-- as far as I can tell, the JS just embeds the video in your browser.
The "best" real way I can think of doing is someone hosting a server in an unblocked country, maybe using VPN(s) or different servers for different sorts of blockage, fetch the video on the server/stream it from YT to webserver to client... It's really stupid and roundabout, and would be no negligable burden for the server-host, but it'd work.
